Registered Nurse & Clinical Team Leader

Registered Nurse & Clinical Team Leader
£30,000-£40,000 DOE + Annual Performance Bonus
Manchester + Community-Based

If you're a Registered Nurse with an active NMC PIN looking for a move away from traditional ward or shift-based nursing, this is an opportunity to use your clinical experience in a role where you can have a much wider impact.
You'll provide clinical leadership across our community-based care services, develop and support our Support Worker teams, oversee clinical quality and help ensure people with complex care needs receive safe, consistent and genuinely person-centred support in their own homes.

This is not a routine shift-based nursing role. It combines clinical leadership, staff development, quality assurance and hands-on involvement within our care packages when your expertise is needed.

Why join us?

  • Use your nursing qualification to influence how care is delivered, rather than simply deliver the next shift.
  • Take genuine ownership of clinical standards, staff capability and quality.
  • Train, coach and develop Support Workers and see the impact directly within people's homes.
  • Help shape complex community care packages from assessment through to ongoing delivery.
  • Work alongside an experienced Registered Manager, Care Co-ordinator and wider management team.
  • Have the autonomy to make clinical decisions, challenge unsafe practice and drive improvement.
  • Annual performance bonus.

About the role
As our Registered Nurse - Clinical Team Leader, you'll be the day-to-day clinical lead across our care delivery.
You'll oversee clinical safety and quality, assess and review service users, develop care plans and risk assessments, lead training and competency assessment, oversee medicines governance and provide visible clinical leadership to our Support Worker teams.
A major part of the role is ensuring our teams have the skills, confidence, training and competency required to provide safe care.
You'll work closely with our Care Co-ordinator / Head of Compliance, who leads operational coordination, rostering and compliance systems, and our Registered Manager, who retains overall regulatory and governance oversight.
The role involves a flexible mix of time in our Manchester office and visiting care packages in the community. Some weeks will require more time with teams and service users, while others will focus more on training, clinical governance and office-based work.
You'll also participate in our on-call rota. Very occasionally, where the normal care team and wider bank have been exhausted, you may be required to support emergency shift cover as a last-resort contingency.
A full UK driving licence is essential.

What you'll be doing

  • Provide clinical leadership and oversight across community-based care packages.
  • Complete clinical assessments for new service users and support the safe set-up of new packages.
  • Write and review person-centred support plans, clinical risk assessments and escalation plans.
  • Lead the training, coaching, induction and development of Support Workers.
  • Assess and sign off clinical competencies and determine when staff can safely undertake delegated healthcare tasks.
  • Provide package-specific supervision and clinical support.
  • Lead medicines governance, including incidents, competency and medication audits.
  • Review clinical incidents, near misses and concerns and ensure learning is embedded into practice.
  • Undertake clinical spot checks and observations within people's homes.
  • Oversee clinical equipment, delegated healthcare tasks and other areas of clinical risk.
  • Work with families, GPs, pharmacies and wider multidisciplinary teams.
  • Provide clinical leadership around safeguarding, Infection Prevention & Control, consent and Mental Capacity.
  • Contribute to CQC readiness, clinical audits, quality assurance and continuous improvement.
  • Act where care or clinical practice is unsafe, including requiring retraining or reassessment where necessary.

What we're looking for

  • Registered Nurse with an active NMC PIN - essential.
  • Recent nursing experience relevant to complex, community or domiciliary care.
  • Strong clinical assessment, risk-management and care-planning skills.
  • Experience supporting people with complex or delegated healthcare needs.
  • Experience training, coaching, supervising or clinically supporting care staff.
  • Confidence assessing competency and making decisions around safe delegation.
  • Good knowledge of medicines management, safeguarding, clinical risk, Infection Prevention & Control and incident management.
  • Confidence challenging poor or unsafe practice and taking appropriate action.
  • Strong written documentation and record-keeping skills.
  • Comfortable using digital care records, eMAR or similar systems.
  • Excellent communication, leadership and organisational skills.
  • Full UK driving licence.
  • A genuine commitment to developing staff and improving the quality of care people receive at home.

Bonus points for:

  • Previous experience as a Clinical Lead, Team Leader or similar.
  • Experience within a CQC-regulated domiciliary or complex-care service.
  • Experience supporting CQC inspection or quality-improvement activity.
  • Formal teaching, competency-assessment, practice-educator or Train-the-Trainer experience.
  • Experience using electronic domiciliary-care management systems.
  • Experience working within an organisation rated Outstanding by the CQC.
