Our client Swindon Council is looking for a Social Worker to join their Learning Disabilities Team.



Job Purpose

To provide high-quality, person-centred social work services to adults with learning disabilities, promoting independence, wellbeing, choice, and inclusion. The post holder will assess eligible needs, develop outcome-focused support plans, undertake safeguarding responsibilities, and work collaboratively with individuals, families, carers, and partner agencies to achieve positive outcomes in accordance with statutory duties.



Key Responsibilities

  • Manage a caseload of adults with learning disabilities, ensuring timely assessment, support planning, and review.
  • Undertake strengths-based assessments under the Care Act 2014, identifying eligible needs and promoting independence.
  • Complete Mental Capacity Act (MCA) assessments and Best Interests decision-making where appropriate.
  • Undertake safeguarding enquiries in line with Section 42 of the Care Act and local safeguarding procedures.
  • Develop person-centred care and support plans that maximise independence and community participation.
  • Carry out risk assessments and implement effective risk management plans while promoting positive risk-taking.
  • Work in partnership with individuals, families, carers, advocates, health professionals, housing providers, and voluntary organisations.
  • Support transitions from Children's Services to Adult Social Care where required.
  • Participate in Continuing Healthcare (CHC) assessments and multidisciplinary meetings.
  • Promote access to education, employment, supported living, day opportunities, and community resources.
  • Prepare reports for funding panels, legal proceedings, Court of Protection, and other statutory meetings where required.
  • Maintain accurate, timely, and analytical case records using the organisation's electronic case management system.
  • Monitor commissioned care packages and review outcomes to ensure they continue to meet assessed needs.
  • Respond to urgent situations and participate in duty arrangements as required.
  • Ensure practice reflects current legislation, statutory guidance, and professional standards.
