Location: Based primarily at HMP Brixton

Salary: £18,049.02 per annum

About The Role

Are you passionate about improving outcomes for people leaving prison and making sure they receive the right support when they return to the community?

We re looking for an enthusiastic and organised Continuity of Care Coordinator to join our substance misuse team at HMP Brixton. This is an exciting opportunity to play a key role in strengthening the journey from prison into community treatment, building effective partnerships and helping ensure people leaving custody experience a safe and supported transition.

Based primarily at HMP Brixton, you ll work across our London services, including HMP Wormwood Scrubs and Heathrow IRC, so some travel will be required. Reasonable travel expenses will be covered.

What you will be doing

You ll be at the heart of improving continuity of care across our London prison services. Your responsibilities will include:

  • Leading monthly release audits, ensuring service users are correctly discharged on NDTMS and that any data issues are identified and resolved.
  • Completing pre-release audits to make sure community teams are fully informed and prepared for an individual's release.
  • Ensuring alert forms and key information are shared with community services so that support can continue without unnecessary delays.
  • Building strong relationships with local authority and community substance misuse teams, developing effective referral pathways and joint-working arrangements.
  • Keeping up to date with local community treatment offers and ensuring prison teams have the information they need to support successful referrals.
  • Establishing and chairing continuity of care forums, bringing partners together to identify challenges, share learning and improve outcomes.
  • Producing accurate meeting minutes, actions and progress updates, ensuring agreed improvements are followed through.
  • Acting as a key point of contact for community prison in-reach workers, helping them overcome barriers and supporting effective pre-release reviews.
  • Leading regional NDTMS and other relevant audits, identifying opportunities to improve data quality and performance.
  • Supporting the organisation of release fairs and events within the prison, helping service users connect with community support.
  • Regularly reviewing and updating the Continuity of Care Best Practice Framework, ensuring it reflects current guidance and good practice.
  • Developing strong community partnerships that improve continuity of care and outcomes for service users.
  • Working collaboratively with Health and Justice colleagues, NHSE and OHID to support the delivery of commissioning projects and initiatives.
  • Sharing best practice, progress and learning with managers and teams across the region.

What we are looking for

We re looking for someone who is passionate about partnership working and committed to making a real difference to people moving from prison back into the community.

You ll ideally bring:

  • Experience of working within substance misuse, health, criminal justice or a related setting.
  • A strong understanding of the challenges people can face when transitioning from prison into community services.
  • Excellent communication and relationship-building skills, with the ability to work confidently with a wide range of partners.
  • Strong organisational skills and the ability to manage audits, meetings, actions and deadlines effectively.
  • Excellent attention to detail, particularly when working with data, records and performance information.
  • The confidence to chair meetings, influence others and drive actions forward.
  • Experience of working collaboratively across different organisations and professional teams.
  • The ability to identify barriers, find practical solutions and continuously look for ways to improve services.
  • A proactive and motivated approach, with the confidence to work independently while also being a strong team player.
  • A commitment to person-centred support, equality and improving outcomes for people affected by substance misuse and the criminal justice system.
  • The flexibility to travel between London sites when required.

This is more than an administrative coordination role. You ll have the opportunity to shape how continuity of care is delivered across London, strengthen relationships between prison and community services, and help ensure people leaving custody have the support they need to move forward.
