Working closely with each individual, you'll identify barriers, develop tailored action plans, and provide practical guidance to help them achieve their goals. Through one-to-one support, mentoring, and group-based interventions, you'll encourage positive behavioural change, improve wellbeing, and help participants build the confidence and skills needed to move forward.

  • Support participants to access housing, wellbeing services, mental health support, substance misuse services, education, training and other community resources.
  • Build positive, professional relationships that encourage engagement and personal development.
  • Work collaboratively with peer mentors, volunteers and partner agencies to achieve positive outcomes.
  • Maintain accurate records using case management systems.
  • Manage risk appropriately and follow safeguarding procedures.
  • Work towards individual and team performance targets.
  • Contribute to continuous improvement by sharing knowledge and best practice with colleagues.

Above all, you'll be someone who can build trust, engage with people from diverse backgrounds, and motivate individuals to overcome challenges and achieve their potential.

Skills and Experience
Essential:
  • Excellent communication and interpersonal skills.
  • Strong relationship-building and engagement skills.
  • Good organisational and time management abilities.
