What you will do:
Undertake Care Act 2014 assessments, reviews, and support planning, ensuring person-centred and strengths-based outcomes.
Manage a caseload of adults with complex needs, including older people, physical disabilities, and mental health conditions.
Lead and contribute to safeguarding enquiries (Section 42), ensuring effective risk management and protection of vulnerable adults.
Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
Person Specification:Qualifications:
Degree in Social Work (BA/MA or equivalent)
Evidence of continued professional development
Registration & Compliance:
Registered with Social Work England
Knowledge of relevant legislation and frameworks including the Care Act 2014 and Mental Capacity Act 2005
Enhanced DBS clearance
