The UK’s Biggest Workforce: Why That Matters For Your Career

According to the latest HMRC PAYE data, Health and Social Work accounts for approximately 15% of all UK employees: more than retail, more than education, and more than finance and technology combined. It is, by some distance, the industry that keeps Britain running.

But here is what that headline figure doesn’t tell you: despite employing more than one in seven working people in this country, healthcare cannot fill its roles fast enough. The gap between supply and demand has never been more significant, and for professionals who know where to look, that gap represents real opportunity.

A Sector That Cannot Stand Still

NHS England recorded approximately 97,500 unfilled substantive posts as of March 2026. While that represents a modest improvement on the previous year, the numbers remain striking: a 6.5% vacancy rate across the service, rising to 9% in mental health and 7% in community services, the very areas where demand from an ageing population is growing fastest.
Among nurses and midwives, there are over 21,000 vacancies. Among secondary care doctors, more than 7,400 posts sit empty, and the British Medical Association notes that even these figures are likely to understate the true picture, given how vacancies are recorded.

The structural challenge is significant. England currently has 3.2 doctors per 1,000 people, against an EU OECD average of 3.9. To reach the European average, England would need 40,000 additional doctors. In some regions, such as the East of England, the ratio falls as low as 2.5 per 1,000.

Social Care: Turning a Corner, But Far From Finished

In adult social care, the picture is more nuanced. The vacancy rate has fallen to 6.2%, its lowest level in a decade, representing around 96,000 unfilled roles on any given day. That is genuine progress, and it reflects a workforce that has grown by 22,000 filled posts over the past year.

Yet even at this decade low, the social care vacancy rate remains approximately three times higher than in the wider UK economy. The domestic workforce is contracting (down 130,000 British national workers since 2020/21), and international recruitment, which had helped offset that decline, has slowed sharply following changes to visa routes.
Most telling of all is the long view: Skills for Care projects that adult social care will need 410,000 additional posts by 2040, driven by the needs of England’s rapidly ageing population. The direction of travel is unmistakable.

The Pay Question: What It Means For You

The HMRC data shows something else worth confronting directly. Health and social work, the UK’s largest employer, sits towards the lower end of the pay spectrum relative to other sectors, with a median annual salary of around £31,000. That reality is not lost on the workforce: 43% of NHS staff report dissatisfaction with their pay, and 29% say they often think about leaving.

But there is an important story within that story. The range of roles, settings, and specialisms across health and social care is vast. Specialist clinical positions, leadership roles, private sector practice, and independent healthcare settings frequently offer significantly different remuneration. The sector’s median does not define any individual’s ceiling.
