The Forward Trust is seeking a Band 6 Community Substance Misuse Nurse to join our team in Thurrock. This is a part-time role, 17.5 hours per week, delivering high-quality, person-centred care to people experiencing substance dependency.
You will work as part of a multidisciplinary team, providing and supervising evidence-based substance misuse interventions. Your role will include triage and assessment, brief interventions, prescribed drug treatment, case management, crisis support, group sessions, and 1:1 work. You will also support access to primary care and mental health services, promoting healthier lifestyles and overall wellbeing.

Conduct comprehensive clinical assessments and contribute to tailored treatment plans.
Work collaboratively with a range of professionals including prison staff, healthcare colleagues, and external support agencies.
Provide leadership within the team and undertake managerial responsibilities where required.
Ensure safe, person-centred, and recovery-focused care for all service users.
Ideal Candidate

A Minimum of 2 years post-registration experience
Evidence of personal development since entering the NMC register
Knowledge of the safe storage and administration of medication
Knowledge and ability to comply with the NMC Code of Conduct
Ability to assess, plan, implement and evaluate patient care and act accordingly if concerns present themselves
Ability and knowledge to carry out risk assessments and management
Strong IT Skills E A Good written and verbal communication skills
Ability to maintain accurate patient records within the standards required locally, the Forward Trust and NMC
Demonstrate the ability to work under pressure
Understand the different roles within a multidisciplinary team and be able to work effectively within this

These roles are working full time ideally standard hours of 37.5 hours per week working 8am-8pm, however there is possibility to work part time if not filled full time, any on shift expenses will be compensated.
In this role you will support service users all the way from Initial triage and assessment, to then latterly formulate the immediate care plan & harm reduction advice. You will also be required to sign post out to additional appropriate support services, then arranging the first clinical or psychological interventions appointment along with ongoing support in a secure setting.
