Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Hospital Discharge Team, based at St George's Hospital, Tooting.
Duties and Responsibilities
Manage an allocated caseload of adults requiring support with hospital discharge and ongoing care needs.
Undertake comprehensive Care Act assessments, reviews, and care and support planning.
Assess individual needs, risks, strengths, and eligibility and arrange appropriate care and support.
Work collaboratively with health professionals to facilitate safe and timely hospital discharge.
Undertake Mental Capacity Assessments and Best Interests decision-making where required.
Identify and respond appropriately to safeguarding concerns.
Work closely with patients, families, carers, health professionals, housing, and other partner agencies.
Promote independence, prevention, and strengths-based approaches when developing support plans.
Maintain accurate case recordings, assessments, reviews, and reports in line with statutory requirements.
Ensure work is completed within required timescales and in accordance with relevant legislation and local procedures.
Working Pattern
WFH, with an expectation to be in the office at least 3 days per week.
Location: St George's Hospital, Tooting
Pay Rate
32 p/h Umbrella
Essential Requirements
Social Work England (SWE) registration.
Social Work qualification.
Previous experience relevant to Adult Social Care, Hospital Discharge, or Hospital Social Work.
Experience completing Care Act assessments, reviews, care planning, and risk assessments.
Knowledge and practical experience of the Mental Capacity Act and safeguarding procedures.
Strong multi-agency working and communication skills.
