Location: Islington, London
Rate: 31.37 per hour
Working Pattern: Hybrid
Hours: Monday-Friday, initially 9am-5pm, with some 10am-6pm shifts. Later moving to Monday-Sunday, 1 in 6 rota

Eden Brown Synergy is currently recruiting for a qualified Occupational Therapist to join the Integrated Discharge Service (IDS) within Islington Adult Social Services.

This is an exciting opportunity to join a dynamic and fast-paced Hospital Discharge Service, supporting Islington residents to return home safely while promoting independence, recovery and positive outcomes.

The Role

As an Occupational Therapist, you will play a key role in assessing risk and designing interventions to support residents at different stages of the hospital discharge pathway.

Key responsibilities include:

Supporting the Discharge to Assess pathway through referral screening and discharge planning.
Participating in the 7-day screening rota, expected to commence after approximately 2 months.
Completing post-discharge assessments and providing one-off therapy interventions.
Assessing home environments and arranging equipment and minor adaptations.
Providing advice on managing daily activities and manual handling.
Developing Reablement Plans and Goals using a strengths-based, outcome-focused approach.
Working closely with Social Workers, Reablement, Health colleagues and wider multidisciplinary teams.
Supporting independence, recovery and admission prevention.

The IDS is co-located with Islington Reablement, providing excellent opportunities for Occupational Therapists to shape Reablement Plans from the outset of a resident's care journey.

What We're Looking For

Qualified Occupational Therapist.
Relevant professional registration.
Experience in adult social care, hospital discharge, community or reablement settings is desirable.
Strong assessment, risk management and clinical decision-making skills.
A person-centred and strengths-based approach.
Ability to work effectively within a multidisciplinary team.
Excellent communication and organisational skills.
