This is a fantastic opportunity for someone with experience supporting vulnerable people who enjoys building positive relationships and helping customers achieve greater independence.
As a Living Well Coordinator, you'll provide person-centred support to customers, carrying out well-being visits, helping people access appropriate services and working alongside colleagues and external partners to ensure customers receive the right support.
