Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Hospital Pathways Team at Sutton Council.
The role involves supporting adults through hospital pathways and discharge planning, alongside safeguarding, Care Act assessments, mental capacity assessments, continuing healthcare assessments, and complex case management.
Duties and Responsibilities
Working Pattern
3 days office-based at Civic Offices, Sutton.
2 days working from home.
Hybrid working.
Pay Rate
32 p/h Umbrella
Essential Requirements
Social Work Qualification - Degree or equivalent.
Social Work England Registration.
Previous experience within Adult Social Care, particularly hospital pathways or hospital discharge services.
Good knowledge of the Care Act 2014, Mental Capacity Act 2005, Best Interests decision-making, and Adult Safeguarding.
Experience completing Care Act assessments, risk assessments, and safeguarding enquiries.
Experience working within multi-disciplinary teams and alongside health professionals.
Ability to manage a varied caseload and work effectively within a fast-paced environment.
Strong communication, assessment, analytical, and report-writing skills.
