The Leaving Care Service provides a statutory service to young people aged between 16-25 years who have been, or are Looked After by the local authority.

The Leaving Care Team is committed to ensuring young people:

The Personal Advisor will play a key role in supporting young people with their transition to adulthood and assist them to live an independent life in their community.

DUTIES AND RESPONSIBILITIES INCLUDE:

• To manage a caseload and keep accurate case records of work with young people and undertake statutory recordings.
• To contribute to Pathway Plan Assessment and provide ongoing assessment promoting young people s full engagement to inform Pathway Planning.
• To undertake assessments of young people resuming or wishing to resume education/ training or presenting with other support needs post 21 years.
• To work with young people to promote welfare and ensure that their views and wishes are considered when undertaking assessments, developing Pathway Plans and reviews.
• To work in a flexible manner to provide advice, practical assistance and support to young people to enable successful transitions.
• To coordinate the provision of services and to take reasonable steps to ensure that the young person makes use of such services
• To identify packages of support for young people living in the community and supervise and monitor the delivery of tasks in accordance with the pathway plan to meet young person s needs.
• To ensure that young people receive appropriate financial support and work with young people to develop their financial management skills with a view to them becoming responsible for their own budget.
• To advise and practically support young people to gain access to appropriate accommodation
• To work in partnership with a range of agencies to ensure young people receive an integrated, holistic response to meet their needs.

ESSENTIAL REQUIREMENTS INCLUDE:

• Substantial experience of working and engaging with young people.
• Experience of working with a variety of different agencies and social care professionals and working in a multi-racial/multi-cultural community.
• Experience of dealing effectively with sensitive and confidential matters, pressure and conflict.
• An understanding of relevant legislation and statutory guidance including Working Together 2014, MSCB Safeguarding Procedures, Domestic Abuse awareness, Children Act 2004, Children and Families Act 2014

THE ROLE:

• To contribute to the operational delivery of an effective Children s Social Care Service as part of a team responsible for safeguarding and promoting the welfare of children and young people who will be leaving or have left care provision, continuously improving outcomes.

• Identify and assess levels of risk and need and take responsibility for reporting risk through the line management structure.
• Identify and report safeguarding issues in line with legislation and local procedures.
• Work co-operatively with colleagues and provide expertise/consultative service relating to young people s experiences in care and after they leave care.
• Work in partnership within the Council and across agency boundaries including statutory, voluntary and local community and faith groups.
• Attend and represent the department at a range of meetings including reviews and case conference meetings.
• Be responsible, as specified in Departmental Guidance, for accurate, timely and up to date data entry on all cases including:
  • data entry on ICS and any other electronic tools or database
  • data required for specific PIs/targets
  • Ensure that client information data is lawfully gathered, accurate, up to date and only divulged in accordance with the Data Protection Act 1998 and the local government common law duty of confidentiality. Failure to apply these duties can lead to the individual or the Department facing court proceedings.

We are currently working with the London Borough of Bexley to find an experienced Qualified Social Worker for their Transitions and Leaving Care Service. This is an excellent opportunity to support young people in care aged 16 and 17 as they prepare for independence, ensuring they have the right plans, support and opportunities in place for a successful transition into adulthood.

This is a case-holding role where you'll manage statutory responsibilities for young people in care, completing assessments and developing comprehensive, person-centred pathway plans before they transfer to a Personal Advisor within the service. Duties

• Managing a caseload of 16 and 17-year-old young people in care as they prepare for independence.
