This is a genuine growth role, with the successful candidate initially taking clinical responsibility for just one package of care in Norfolk. The Role
As the Learning Disabilities Clinical Lead, you'll provide clinical leadership and oversight for specialist community care, initially taking responsibility for one package in Norfolk.

You'll work closely with the individual, their family, frontline care team and multidisciplinary professionals to ensure the package delivers safe, effective and person-centred care.

As the service develops, you'll support the assessment and mobilisation of new packages, train and develop frontline teams and maintain high standards of clinical governance across your caseload.

The hybrid structure provides 1-2 days working from home each week, alongside time spent within the community supporting packages and frontline teams.

This is an opportunity to combine hands-on clinical leadership with service development, clinical governance, staff training and complex care management, while helping shape the continued growth of services across Norfolk.

Key Responsibilities
  • Take clinical responsibility for an initial complex community care package in Norfolk.
  • Complete comprehensive clinical assessments, care plans, risk assessments and regular clinical reviews.
  • Provide clinical leadership and oversight to Nurses and Support Workers delivering care in the community.
  • Deliver clinical supervision, competency assessments, practical training and ongoing staff development.
  • Support the assessment and safe mobilisation of new complex care packages as the service grows.
  • Ensure care packages remain safe, effective, person-centred and clinically robust.
  • Lead safeguarding investigations, clinical audits, incident reviews and root cause analysis where required.
  • Maintain high standards of clinical governance, quality assurance and regulatory compliance.
  • Develop and review clinical policies, procedures and best-practice guidance.
  • Work collaboratively with families, commissioners, Integrated Care Boards (ICBs), Local Authorities and multidisciplinary professionals.
  • Promote best practice around Positive Behaviour Support (PBS), Mental Capacity Act (MCA), Best Interest decision making, safeguarding, trauma-informed care and person-centred approaches.
  • Contribute to the continued development of specialist learning disability, autism and mental health services across the region.
About You
To be considered for this Learning Disabilities Clinical Lead position, you'll need:
  • Registered Learning Disabilities Nurse (RNLD) or Registered Mental Health Nurse (RMN) with active NMC registration.
  • Strong experience supporting adults with learning disabilities, autism, mental health needs and complex behaviours.
  • Experience within community complex care, supported living, community nursing, mental health services or another relevant community-based clinical environment.
  • Strong clinical assessment, care planning and risk assessment experience.
  • Experience delivering clinical supervision, competency assessments and training to frontline teams.
  • Good knowledge of Positive Behaviour Support (PBS), Mental Capacity Act (MCA), safeguarding and clinical governance.
  • Confidence working autonomously and making appropriate clinical decisions within community settings.
  • Strong leadership, communication and stakeholder management skills.
  • Ability to work effectively with families, commissioners and multidisciplinary professionals.

About the Role

Determine the appropriate staff skill mix and clinical competencies required for individual packages, providing clinical direction and support to Healthcare Assistants, Support Workers and Nurses working within services. Undertake clinical assessments, reviews and risk assessments and ensure appropriate care and support plans are in place.

  • Provide advice and clinical oversight where individuals present with mental health needs, learning disabilities, autism, behaviours that challenge or complex care requirements. Support the management of incidents, safeguarding concerns, medication issues and other clinical risks, and lead or contribute to investigations, incident reviews and lessons-learned processes where required.

  • Carry out quality assurance visits, spot checks and clinical audits, assess and sign off staff competencies relevant to the individual's care and identified risks, and provide clinical supervision, coaching and guidance to staff. Identify gaps in staffing, competency, training or clinical provision and implement appropriate actions.

  • Work collaboratively with families, commissioners, social workers, case managers and other professionals. Escalate significant clinical, safeguarding or operational risks through the appropriate governance structures, ensuring clinical documentation, risk assessments, care plans and records are accurate, current and compliant.

  • Promote safe practice in accordance with NMC standards, safeguarding requirements, CQC expectations and relevant legislation and guidance. Contribute to the continuous development of Medgen's clinical governance and crisis response processes.

What We're Looking For

We are looking for a confident registered nurse who is comfortable working in complex, unpredictable and fast-moving environments.

You must have:

  • Current and valid NMC registration as one of the following: Registered Mental Health Nurse (RMN), Registered Adult Nurse (RN/RGN), or Registered Learning Disability Nurse (RNLD/RLDN).
  • Relevant clinical experience within mental health, learning disabilities, autism, complex care, challenging behaviour or other high-risk care environments, alongside experience of assessing clinical risk and making sound professional decisions.
  • Experience working with individuals whose needs may change rapidly or who require enhanced support, including providing clinical direction to care and support staff and conducting staff competency assessments and clinical supervision.
  • Strong understanding of safeguarding, incident management and escalation, with excellent clinical judgement and the ability to remain calm and decisive during crisis situations.
  • Strong communication skills and the ability to work effectively with families, commissioners and multidisciplinary professionals, while working autonomously and recognising when escalation is required.
  • Strong organisational skills and the ability to prioritise competing demands.
