Social Worker required for work within the Adults Community Mental Health Team in the Devon area. This post will be based in Exeter, which has good transport links to the M5 and A38. Close to the coast and in an area of outstanding national beauty. 37 hours a week with rates up to 31-35ph for experienced candidates.

Location -
Home working, with Monthly attendance in person to supervision and team meetings there is the opportunity to work from DCC buildings and some health facilities.

Working in the community with adults aged 18yrs and over, however we do start to work with those transitioning to adulthood from Childrens services at the age of 17 years onwards, with Childrens services retaining responsibility until the person turns 18 years of age.

The successful applicant should be able to undertake Care Act Assessments, Mental Capacity assessments & Best Interest outcomes. Experienced social worker with a good track record to be able to operate from a Strengths Based perspective, demonstrating Least Restrictive Practice.
Support the team particularly our Social Care Assessors. Effectively prioritising their day to day case management activities. Confident in working with professional partners across statutory and voluntary sectors, in an MDT approach.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK

Our client Yorkshire Collaboration is looking for a Senior Social Worker to join their Integrated Front Door team.

We are seeking 2 full time senior social workers for our Children's Integrated Front Door. These are "over-establishment" roles to provide additional capacity for our busy front door. The posts are for an initial 8 week period, however there is likely to be an opportunity for these to be extended beyond this date. The successful candidates will have worked previously within a MASH/Screening Team and will be able to work at pace. The candidates will have a clear understanding of thresholds, be able to undertake risk assessments, complete safety plans and analyse information from multiple sources to inform plans and ensure children receive the level of service/help they require at the right time. Previous experience of working within a signs of safety framework and using Mosaic is preferred.

Hybrid working arrangements are in place - currently 3 days in the office and 2 days WFH each week.

• Maintain accurate, up to date records safely and confidentially in accordance with the Council's policies and procedures. Produce succinct, well-structured records and reports, clearly recording and reporting analysis and judgements.
• Pro-actively engage with colleagues and a range of organisations to identify, assess, plan for and support the needs of children, young people, families/carers in order to promote positive change and independence, whilst demonstrating confident and effective judgement about risk to children and young people.

Role portfolio:

The Adult Social Care (ASC) Multi-Agency Safeguarding Hub (MASH) & Early Help & Targeted Support Team will deal with new contacts that could range from advice and guidance, concerns for welfare and safeguarding concerns. To make every contact count to enable adults to have choice and control, maintain their independence, wellbeing, safety and mitigation of avoidable risk or harm.

Within the ASC MASH & Early Help & Targeted Support Team, information from different agencies will be collated using a strengths-based conversation model to inform what proportionate action may be required. As a result, the agencies using an multi disciplinary team (MDT) model will be able to act in a timely, coordinated and consistent way to enable adults to achieve their desired outcomes to live their best lives.

The MASH & Early Help & Targeted Support Team involves representation from the City Council, Police, Health and other partners working together in the same location. Virtual links will exist to other services and agencies, such as Primary Care, VCSE, probation, housing and early help services.

A key facet of the role will be supporting and integrating MASH with the delivery of the Early Help workstream of Better Outcomes Better Lives (BOBL) programme. BOBL is Adult Social Care's central long-term transformation programme, of practice-led change focused on supporting the people of Manchester to achieve better life outcomes with less dependence on formal care.

The aim of the Early Help workstream is to develop a new integrated Adult's Front Door model where professional capacity in the form of the Early Help & Targeted Support Team will work with the ASC Contact Centre function to ensure that the right tools, skills and knowledge are available to support the best outcomes by making every contact count.

Key Responsibilities

Supervision & Leadership

• Provide supervision and practice advice to Case Workers and Senior Case Workers.
• Make professional decisions on complex cases and triage safeguarding referrals.

Support & Advice

• Manage referrals, resolve queries, escalate safeguarding concerns, and prepare clear reports.

Relationship Management
