We are recruiting for a Brokerage Officer to join the Care Navigation Team, supporting adults aged 18+ and young people preparing for adulthood across Lancashire. About the Role
The Brokerage Service is responsible for planning, sourcing, negotiating and arranging care and support packages that meet the individual needs of adults and young people transitioning into adult services. This includes arranging specialist placements, coordinating packages of care, and working with approved providers, framework agreements and block contracts. As a Brokerage Officer, you will work alongside individuals, recognising them as the experts in their own lives. You will help identify the most appropriate support by promoting independence, making the best use of community resources, and ensuring services are both person-centred and financially sustainable.
