Job summary

We have an exciting opportunity for a Specialist Occupational Therapist at Buckinghamshire Healthcare NHS Trust to join the Rapid Response & Intermediate Care (RRIC) Teams across Buckingham and Marlow. In this rewarding community-based role, you will be responsible for delivering active case management programmes for patients with complex needs, ensuring person-centred interventions are tailored to meet individual requirements. You will work closely with Advanced Community Practitioners to support workload allocation across the team, coordinate care plans, and contribute to the delivery of high-quality, timely care. Main duties of the job

As a Specialist Occupational Therapist, you will be responsible for proactively identifying individuals who are frequent users of community and primary healthcare services and those at risk of unplanned hospital admissions. You will support the management of referrals into the service, ensuring patients are appropriately triaged and directed to the most suitable interventions. You will utilise your specialist clinical expertise to evaluate functional needs, deliver targeted therapeutic support, and monitor patient progress. In addition, you will oversee a dedicated caseload of individuals living with long-term conditions, helping them maintain independence, reduce avoidable healthcare utilisation, and achieve improved health and wellbeing outcomes.

Person Specification & Qualification

• HCPC registered Occupational Therapist

• Full UK driving licence, own vehicle, and evidence of ongoing CPD. Evidence of ongoing CPD.
