About the role

The Clinical Nurse Specialists play an essential role in providing high-quality, person centred care for individuals facing life-limiting illnesses.

This is an exciting opportunity for an experienced Palliative Care Nurse who wants to work in a supportive environment where care of the patients comes first.
Your main duties will be to provide direct assessment, support and advocacy to service users and carers dealing with complex, distressing and life-limiting illnesses.

The successful applicant will be responsible for providing support to patients, families, and healthcare professionals, ensuring they receive timely advice and guidance. The role will be primarily based in Berkshire, with travel to the In-Patient Unit in Reading and South Oxfordshire, working a combination of day and night shifts.

Responsibilities will include responding to telephone calls, offering appropriate clinical advice, and supporting the wider team to deliver high-quality care across both settings.
The successful candidate will also be expected to have a strong interest in completing their Non-Medical Prescribing (NMP) course as part of their ongoing professional development.

In addition:

You will collect, collate, evaluate and report information, maintaining accurate service user records.

About you

The successful applicant will play an active role within the team. The organisation prides itself on giving their patients the best possible quality of life by providing a holistic and individualised service.
The ideal candidate will have significant experience of working in Palliative and End of Life Care with evidence of additional, role specific qualifications and professional development.

Minimum Essential Criteria:

NMC registered with an interest in working towards their Non-Medical Prescribing qualification
Previous Palliative care Community experience
Ability to support adjustments for diverse patient needs
Skilled in accessible communication with patients and families
Commitment to inclusion and patient advocacy

Desirable Criteria:

Advanced communication training (e.g. Sage & Thyme)
Experience with symptom management in complex needs

The organisation provides a thorough induction with supernumerary time to settle into the role and full training/competency assessment will be provided.
There is plenty of scope to further develop your own skills in specialist palliative care, and planned career progression through their education programme and development framework is encouraged.
