Our Community Teams support adults facing a range of complex challenges associated with substance use. Based within local community hubs, you'll play a vital role in helping people achieve their recovery goals through:

• Case management and care coordination• One-to-one psychosocial interventions• Harm reduction advice and support• Monitoring and supporting prescribed treatment alongside clinical colleagues• Working in partnership with a range of agencies to develop holistic recovery plans that promote long-term wellbeing, independence, and resilience

We're looking for enthusiastic, compassionate individuals who are committed to supporting people to create positive change.You will hold one of the following:

• A relevant degree such as Psychology, Sociology, Social Work, or a related discipline; or• A Diploma in Health and Social Care (or equivalent qualification);• A Level 3 qualification in Tackling Substance Misuse (or equivalent)
