Overview

We are seeking compassionate and dedicated Reablement Support Workers to join our diverse and inclusive team. As a Reablement Support Worker, you will play a vital role in helping individuals regain independence and improve their quality of life through personalised care and support, as well as ensuring safe and timely transportation to and from appointments and community activities.



Responsibilities

  • Provide practical and emotional support to individuals in their own homes or community settings.
  • Assist clients with daily living activities, encouraging independence and confidence.
  • Safely transport clients to appointments, social activities, and community resources.
  • Ensure vehicle cleanliness, maintenance checks, and comply with all road safety regulations.
  • Work collaboratively with healthcare professionals to deliver agency-compliant care plans.
  • Monitor and report any changes in client's health or needs, following safeguarding policies.
  • Ensure all care practices adhere to relevant legislation and organisational standards.
  • Maintain accurate and confidential records in line with agency compliance requirements.
  • Address any concerns or incidents, including unauthorised activities, according to protocols.
  • Plan routes efficiently to optimise time and resources.


Qualifications

  • Previous experience as a Care Support Worker, Reablement Support Worker, or similar role is preferred.
  • Valid driving license with a clean driving record.
  • Experience driving clients safely, including those with mobility or other assistance needs.
  • Understanding of agency compliance standards and care regulations.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to promoting equality, diversity, and inclusion in the workplace and care delivery.
  • Relevant certification or willingness to undertake training in reablement or social care.
  • Basic literacy and numeracy skills required.
  • DBS check and other statutory clearances as applicable.


Day-to-Day

  • Engage with clients to support their daily routines and personal goals.
  • Safely drive clients to appointments, social events, and community activities.
  • Conduct regular vehicle safety and maintenance checks.
  • Attend team meetings and training sessions to enhance skills and knowledge.
  • Complete documentation accurately and promptly using digital or paper systems.
  • Build trusting relationships with clients, respecting their choices and cultural backgrounds.
  • Adapt support strategies to meet the evolving needs of each individual.
  • Respond promptly and appropriately to any incidents of unauthorised activity, ensuring client safety.
  • Collaborate with colleagues and external agencies to provide holistic support.
