The Core ResponsibilitiesAs a Senior Social Worker within the Connecting Care Team, you will use your expertise to promote independence and wellbeing by:
Executing Statutory Duties: Completing robust, evidence-based Care Act Assessments, Support Plans, and complex Reviews in strict alignment with the Care Act 2014.
System Navigation: Utilizing CareDirector daily to accurately record case notes, update assessments, and manage workflows seamlessly.
Providing Senior Oversight: Utilizing your advanced knowledge to handle complex, multi-agency cases and mentoring junior staff or student social workers where required.
Because this is a senior, fast-moving team, the hiring manager requires candidates who meet the following strict criteria:
CareDirector Expertise: You must have recent, direct experience using the CareDirector system. (Please ensure this is clear on your CV, as the authority cannot provide system training for this contract).
Professional Credentials: A recognized Social Work qualification and an active registration with Social Work England.
Care Act Knowledge: Deep, demonstrable experience implementing the Care Act 2014 within a statutory adult social care setting.
Mobility: A valid UK driving license, access to a vehicle, and business insurance for community-based visits.
Competitive Pay: A premium rate of £35.00 per hour for experienced adult social care practitioners.

The care home is a 82 bed, modern home you will be tasked with assisting with the resident s care plan, supervising the caring team, delivering the highest standards of personalised care to residents. Ensure that at all times residents are treated with respect and dignity, and that individual s rights to privacy, dignity, independence and choice are met.
Key Responsibilities
• To provide the highest level of personal care and attention to residents, following individual care plans carefully and ensuring that all contact is polite, friendly, warm and supportive.
• To act as a key worker for a named group of residents, ensuring that these responsibilities are carried out in full (as defined in the training manual).
• To participate in the formulation and maintenance of individual care plans for each resident and administer medication to residents as prescribed, accurately maintaining appropriate records for both.
