The Role

We are currently seeking a Rehabilitation Support Worker to provide structured evening support to a 26-year-old male with an acquired brain injury and polytrauma. The focus of this role is on supporting routines, and providing social and emotional support to wind down the day in a positive and safe way.

The client is friendly, with a great sense of humour. He enjoys listening to music, DJing, darts, and fishing. The support role involves helping him to plan and prepare meals, tidy up, and encourage structure and positive routines to promote wellbeing.

Key Responsibilities:

  • Meal preparation, encouraging independence and healthy choices.
  • Facilitate and promote engagement in activities, including watching films or playing darts.
  • Support to complete household tasks where appropriate.
  • Provide emotional and social support to help the client unwind after the day.
  • Monitor and report any changes in mood, motivation, or physical wellbeing to the case management team.
  • Maintain accurate and detailed shift notes.
  • Liaise with the case manager and therapy team, following guidance and the rehabilitation plan.
  • Advocate for the client when necessary.
  • Uphold safety, dignity, and privacy at all times.
  • Attend supervision and training sessions as required.
  • Transport to and from appointments and commitments.
  • Light cleaning duties.

They should preferably have experience supporting clients with communication and mobility difficulties, as the client can only speak a few words, and his support worker will need to be skilled in interpreting in order to communicate effectively.

The RSW should be able to support mobility, and accessing indoors and outside environment safely. The RSW should be able to motivate the client, and help him to engage in social and leisure pursuits both at home, e.g. board games, card games, as well as therapy exercises from Physio and SALT.

The RSW should also be able to research, plan and organise trips and excursions to the local community to support the client in engaging with activities to improve and enhance quality of life. Ultimately, accessing disabled fishing or sports options would be great. The client also wants to lose weight through activity and exercise, and so the successful candidate should be confident in advising and supporting self control, and motivating the client.

The Role

Our client is a 22-year-old male, and is a Deaf BSL British Sign Language user. He loves football, and so someone with an interest in football would be a bonus!

We are looking for someone who can go out with the client socially, as well as someone who can support him to access clubs, sports, activities etc. and develop hobbies/interests.

The client also wishes to develop his confidence and independence in the hearing world, and develop skills such as money management.

The successful candidate MUST be fluent in British Sign Language (BSL) and we are in search of someone who uses BSL (British Sign Language) as their primary language, regardless of whether they are Deaf, hard of hearing, or have other hearing impairments or have an understanding of Deaf culture and the importance of Deaf identity.
