Are you passionate about supporting people to improve their health, wellbeing and recovery outcomes?

The Forward Trust is seeking a compassionate and motivated Health and Wellbeing Practitioner to join our integrated healthcare team at Heathrow IRC on a part-time basis (21 hours per week). You'll support detainees with substance misuse, low-level mental health needs and wider wellbeing concerns, helping them build resilience and prepare for the next stage of their journey.

What you'll be doing

Working as part of a multi-disciplinary team, you will:

  • Complete assessments, risk management plans and recovery plans
  • Manage a caseload and deliver both 1:1 and group interventions
  • Support individuals with substance misuse and mental health needs
  • Develop release, discharge and continuity of care plans
  • Promote wellbeing services and signpost to relevant support
  • Contribute to multi-disciplinary reviews and daily team meetings
  • Maintain accurate case records and reports
  • Provide transitional support to individuals preparing for release or deportation

Requirements

  • Experience within reablement services or short-term pathways is highly desirable
  • Strong understanding of strengths-based practice, risk management, and safeguarding
  • Ability to manage a dynamic caseload and work independently
  • Excellent communication and partnership-working skills
