Adult's Social Worker - Integrated Care Team

Our Client, a Greater Manchester council, is seeking an Adult's Social Worker to join their Integrated Care Team.

Responsibilities of this role:

Undertake strengths-based assessments under the Care Act 2014
Develop and review person-centred support and care plans
Work collaboratively within multidisciplinary integrated care teams
Coordinate care with health professionals, voluntary sector, and community services
Manage a varied caseload, including complex and high-risk cases
Safeguard adults at risk in line with statutory duties and local procedures
Support hospital discharge planning and prevent unnecessary admissions
Promote independence, choice, and wellbeing for service users
Complete mental capacity assessments and best interest decisions
Maintain accurate, timely case records and statutory documentation
