What you will do:
Conduct thorough assessments of individuals with learning disabilities under the Social Services and Well-being (Wales) Act 2014, ensuring person-centred outcomes.
Manage a diverse caseload, including complex cases and safeguarding concerns, using your professional judgement to support vulnerable adults.
Develop and regularly review care and support plans, promoting independence and community participation.
Lead and participate in safeguarding enquiries, implementing effective risk management strategies.
Person Specification:
Qualifications:
Degree in Social Work (BA/MA or equivalent)
Evidence of continued professional development
Registration & Compliance:
Registered with Social Care Wales
Knowledge of relevant legislation including the Social Services and Well-being (Wales) Act 2014 and the Mental Capacity Act 2005
Enhanced DBS clearance
Familiarity with Safeguarding Adults procedures in Wales
