What you will do:

Coordinate care for complex cases under the Care Programme Approach (CPA), ensuring tailored support for each service user.
Develop collaborative, person-centred care plans aligned with individual needs and goals.
Conduct comprehensive risk assessments and implement effective risk management strategies.
Safeguard vulnerable adults using legislation including the Mental Health Act, Care Act, and Mental Capacity Act.
Carry out holistic mental health and social care assessments, including Care Act assessments.
Work closely with carers, families, and wider support networks to deliver comprehensive care.
Build strong partnerships with statutory and voluntary agencies to enhance service delivery.
Liaise with inpatient units to ensure continuity of care across services.
Maintain accurate, up-to-date records and produce high-quality reports.

What we're looking for:

Strong experience supporting adults with psychosis and complex mental health needs.
Excellent organisational skills with the ability to manage and prioritise a varied caseload.
Sound knowledge of relevant legislation and safeguarding practices.
